  • Paper records (if applicable): All paper notes prior to October 2024 will be stored in a locked filing cabinet under a personal file for your name.
  • Electronic devices used to access personal data are password-protected and have two-factor authentication protection
  • We retain medical records for as long as required by legal or insurance obligations, after which they will be securely shredded, and digital records deleted

  • We are legally required to do so

Subscribing through the online automated process is done at your discretion. Some subscriptions may be manually processed through prior agreement, written or verbal, with you.

Email marketing campaigns published by this website or its owners may contain tracking facilities within the actual email. Subscriber activity is tracked and stored in a database for future analysis and evaluation.
